Nurses are available for in-person NYS Patient Review Instrument (PRI) & SCREEN/PASRR assessments in all 62 counties of New York State (NYS) including all 5 boroughs of New York City (NYC).

 

Assessments are completed in community-based residences and facilities including private residences, group homes, assisted living facilities, and skilled nursing facilities.

Accommodations are made for out-of-state requests.

Physician Rx is NOT required.

We are partnered with several Medicaid Managed Long-Term Care (MLTC) plans that will cover costs associated with NYS PRI & SCREEN/PASRR (prior approval and authorization is required).

* Please be advised, we do not have restrictions in place regarding COVID-19 and/or COVID-19 vaccination. We will gladly schedule an in-person assessment regardless of the client's vaccination status.

 

* Social distancing and wearing of face masks/face coverings is recommended, but not required. 

* Requests for telehealth/virtual appointments are considered on a case-by-case basis (via Zoom video conference, FaceTime, or WhatsApp).

The Patient Review Instrument (PRI) is an assessment tool developed by New York State Department of Health (NYSDOH) to assess the physical, medical, and cognitive characteristics of an individual seeking skilled nursing facility/nursing home admission. It also includes information regarding specialized services they may receive, such as physical therapy and/or occupational therapy.

The SCREEN/PASRR is an assessment tool developed by NYSDOH to determine an individual's potential to be appropriately cared for in a setting other than a Residential Health Care Facility (RHCF). It also includes information regarding dementia, Alzheimer's disease, mental illness (MI), developmental disability (DD), intellectual disability (ID) or a related condition (RC).
